Appointments Policy

Welcome to K. Carrier Family Care, LLC! 

We are committed to providing personalized, high-quality care to every patient. 

To help us serve all patients efficiently, we ask that you please review and follow our appointment policies:

  

Scheduling Appointments

  • Appointments can be scheduled by calling our office at 337-279-5743.
  • We conveniently offer MyRXNT patient portal designed for patients to manage medical information, prescriptions, schedule appointments, pay bills, and navigate your health journey in one simple place. 
  • We offer same-day and next-day appointments for urgent needs when available.
  • New Patient Visits, Routine Visits, Wellness Exams, and Follow-ups should be scheduled in advance to ensure availability.

Pre-appointment Preparation

  • Register in MyRXNT Patient Portal
  • Complete all intake and consent forms

Appointment Arrival

  • Please arrive 15 minutes early to complete any necessary paperwork and check-in procedures.
  • Bring a valid photo ID, your insurance card (if applicable), a list of medications, and any relevant medical records to each visit.
  • If you arrive more than 15 minutes late, you may be asked to reschedule to avoid disrupting other scheduled appointments.


Cancellations and No-Shows

  • If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance.
  • Missed appointments or cancellations with less than 24 hours' notice may result in a missed appointment fee.
  • Repeated no-shows or late cancellations may lead to limited scheduling options or dismissal from the practice.


     New Patient Appointments

  • New patients are asked to complete forms in MyRXNT Patient Portal Application prior to their first visit. 
  • A valid photo ID and insurance verification (if applicable) are required at your first appointment.


     

Walk-In Policy

  • We are a by-appointment practice to ensure each patient receives the appropriate time and care.
  • Limited walk-in availability may be offered for urgent needs, but we strongly recommend calling ahead.


     

After-Hours and Emergency Care

  • For medical emergencies, please call 911 or go directly to the nearest emergency room.
  • For urgent concerns after hours, you may leave a message with our answering service, and a provider will return your call as soon as possible.
  • Routine medication refills, referrals, and appointment scheduling must be handled during regular office hours.


     

Insurance and Payments

  • Please verify insurance coverage prior to your appointment.
  • Copayments, deductibles, and any outstanding balances are due at the time of service.
  • We accept cash, major credit cards, and [other payment options].

Prescription Refill Policy

Your Health and Safety are Our Top Priorities

To ensure you receive the appropriate medications without interruption, we ask that you review and follow our prescription refill policy:

General Refill Guidelines

  • Prescription refills are only provided for patients actively under our care who have been seen within the past [6 or 12] months, depending on the medication.
  • Routine medication refills must be requested at least 3–5 business days before you run out of medication.
  • Refill requests are processed Monday through Friday during regular business hours. Requests made after hours, on weekends, or on holidays will be reviewed the next business day.

   

How to Request a Refill

  • Through Your Pharmacy: Ask your pharmacy to send an electronic refill request to our office.
  • Patient Portal: Submit a refill request online through our secure MyRXNT patient portal.
  • By Phone: Call our office at 337-279-5743 and leave a detailed message with your name, date of birth, medication name, dosage, and preferred pharmacy.

Medications Requiring Office Visits

  

For your safety, certain medications will not be refilled without an in-person evaluation. These include:

  • Controlled substances 
  • Medications for chronic conditions not recently evaluated
  • Antibiotics (generally not refilled without a new assessment)

  

Controlled Substances

  • Refill requests for controlled medications require strict adherence to federal and state regulations.
  • These medications may require a scheduled office visit for renewal.
  • Early refill requests for controlled medications may not be honored.

    

Late Requests and Emergency Refills

  • Please plan ahead. We are not responsible for missed doses due to late refill requests.
  • Emergency refill requests may incur an additional fee if outside of normal      processing times.

     

Changes in Medication

  • Changes to medication dosages, new prescriptions, or restarting a medication after a gap may require an in-person visit.
  • We do not prescribe medications for new conditions without a proper evaluation.

     

Pharmacy Errors

  • If a pharmacy error occurs (incorrect medication, dosage, or missing refill), please contact your pharmacy first.
  • If the error is on our end, we will promptly correct it.
